To Assess Your Loved One’s Level of Care
At The Pathways at Warrington, we understand that it’s a difficult decision to move your parent or loved one out of their home and into a Personal Care or Memory Care community. We also understand that some of the most challenging issues in doing so are simply assessing the level of care they require. These assessment checklists are here to help you determine if mom, dad, or another loved one require daily personal assistance. On this page, you can find one checklist for a parent or loved one who may require personal assistance. And one for a parent or loved one who has been diagnosed with dementia.
